How to become a CRIS Web Forms participating site.

Try entering and saving an AD-421 form using the TEST site by simply selecting TEST as your site (the password is "test"). This should verify that your Web browser will work with the programs (older browsers may not).

Also select the "Administrative functions" link to see what the Administrative user needs to do to manage the process. The administrative password for the TEST site is "admtest".

Then if you decide to use the Web Forms, contact Sue Lang (slang@zoo.uvm.edu).

Provide the name and email address of the person who will be your site contact. This person will be responsible for managing the process, and must be willing to check email regularly.

Give the CRIS code and name of your site as you want it to be displayed in the list of participating sites.

People have asked about training for this. The only training that would be helpful is to know something about the CRIS system. We are trying to provide as much information about CRIS as possible on the Web Forms site. There is quite a lot to it, so there is no getting around this, it WILL take some time to read what is there and follow the links provided, especially if you are new to the CRIS system. You should always let us know if there is information that you need that you do NOT find there.
To use the Web forms does require that you know something about using a Web browser, and the more familiar you are with how browsers work, the better. This is training you can get locally almost anywhere these days.
