AD-419 CRIS Web Forms Testing

The AD-419 CRIS Web Forms programs have been developed and are available now to participating sites who would like to test them. If interested, please email Sue Lang,

The AD-419 CRIS Web Forms will be available to ALL participating sites the end of September.

1998 CRIS Checklists

We expect to have the 1998 CRIS Checklists available to all CRIS sites from the CRIS Forms Assistance website the end of September.

If you would like to become a participating site to use the CRIS Web Forms for 1998 reporting, please review the information on "How to become a participating site".

All participating CRIS Web Forms sites are reminded to download any files that you want to keep for your records or site use prior to the end of September. 1997 Checklist files will become unavailable at the time we cutover to the 1998 checklists.

Submitting AD-421 Reports for mid-year terminations.

The CRIS office sends out requests for AD-421 Termination Reports for grants that have reached their termination date. If the grant is terminating and is a project that was on your 1997 CRIS checklist, you can submit this report using the CRIS Web Forms. Follow the
