To Enter Data for CRIS using Web forms

Help with Web Browser basics

	Select your site by clicking on the radio button directly under the words "Select the site where your information is to be collected:". If you are a participating site but a radio button is not listed, you need to contact your site administrator to find out if data entry for your site is currently allowed. NOTE to Site Contacts: Your site only appears with a radio button in the list if you have enabled these forms in the Administrative Function Edit site information.
	Type in your site password. If you do not know your site password contact your CRIS Site Administrator or Department Representative.
